AmeriCorps is seeking a full-time Community Health Worker to join their team in Montrose, Colorado! The Community Health Worker will work in and with the community to address social determinant of health needs, increase access to care, and improve whole person health for older adults.

Salary: *Stipend for housing*

Responsibilities Include:

  • Coordinate with local providers, programs, and social service agencies to identify and provide community care navigation while meeting the benefits and resources needs of the clients to be helped.
  • Manage and perform tasks related to Medicaid and other public benefits applications, benefit evaluations and appeals.
  • Facilitate health education and behavior change strategies with individuals and groups
  • Develop and/or coordinate on-site health and wellness programs
  • Engage clients in on-site or -in-home health and wellness activities
  • Support clients during hospital stays and/or doctor’s appointments
  • Coach and teach clients as necessary to use access and/or use internet, technology, and/or personal devices in order to meet their use priorities and health and wellness goals. This may include both in person, phone, and online interactions, as well as referral to sources of additional digital literacy skill training.
  • Collaborate with VOA Staff and all other community health care organizations or programs and support services’ Admissions and Marketing Directors, Social Service Directors, other management staff to ensure referrals are communicated timely and effectively to best serve the client to gain entry into outside health care and support services programs.
  • Utilize client and service data systems to facilitate intake and pre-assessments for eligibility to programs and services. Maintains accurate and complete documentation in a timely fashion in the database; maintains and revises client records and service documentation based on client feedback and communications.
  • Educate and communicates about AdvantAge Health Resource Center services and offerings, events, programs, etc. to community referral sources, health care professionals, other professionals, community groups and organizations to develop strong referral network and brand recognition.

Required Qualifications:

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of reliable transportation.
  • High school graduate or equivalent education and minimum 1 year experience and/or education in health or social services environment
  • Experience in health care and at risk seniors.
  • Ability to establish trust, listen, and assist clients with defining their health-management issues, while helping them identify possible solutions
  • Excellent verbal and written communication skills.
  • Excellent public speaking skills for large and small groups.
  • Excellent time management and organizational skills.
  • Current state driver’s license, proof of auto insurance and consistent and reliable usage of a motor vehicle.

Preferred Qualifications:

  •  2-3 years of experience in a health or social services environment.
  • Knowledge base of Delta and Montrose counties community resources.
  • Knowledge of customer service and marketing principles and practice.
  • Able and desire to learn and teach basic technological concepts related to internet services, computer and device characteristics, online services, and applications
