Must have chauffeur license.

Hours: Full-Time, days, occasional Saturdays

Must have experience working with the frail and elderly as a job, taking care of a family member or volunteer.

OBJECTIVE: Under the supervision of the Transportation Lead is responsible for the safe and timely transportation of Senior CommUnity Care (SCC) participants. Responsible for communicating any safety issues.

ESSENTIAL FUNCTIONS:

1. Responsible for Senior CommUnity Care passengers during transport, providing care in a manner which ensures participant satisfaction.

2. Follows assigned schedule, makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to dispatch.

3. Operates equipment safely and in compliance with vehicle licensing regulations.

4. Maintains and completes daily required records i.e., Pre- and Post-Trip Inspections, gasoline purchases.

5. Immediately reports, any vehicle or cell phone operating problems, malfunction, or incidents out of the ordinary routine.

6. Transports participants to and from medical/specialist appointments, and activity outings, as assigned by the Interdisciplinary Team via the plan of care.

7. Transports meals, durable medical equipment, pharmacy, and any other items as deemed necessary for participant.

8. Provides physical assistance to each participant when needed as he/she boards and debarks from the program’s vehicles.

9. Maintains the vehicle in a neat and clean condition. Ability to use automated car wash.

10. Ensures vehicle equipment is maintained and operating within manufactures recommendations, replacing supplies as needed.

11. Functions as a member of the Interdisciplinary Team when appropriate.

12. Communicates with Transportation Lead daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests.

13. Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.

14. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

15. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

16. Participates in and supports Quality Improvement Initiatives.

17. Participates in continuing education classes and any required staff and training meetings. Maintains any required certifications or licenses.

18. Performs other duties as assigned.

Job Requirements

Must have valid Michigan Chauffeur drivers license.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. Must have a valid driver’s license, proof of insurance and have means of transportation.

3. Must pass background check and drug screening.

4. Education: High school diploma or equivalent required;

5. Valid Drivers License in state of hire.

6. CDL Class P license or Chauffer License. upon hire required if location utilizes vehicles requiring

7. Experience: Prefer a minimum of one year experience in working with the frail or elderly population.

8. Skills and Knowledge:

• Knowledge of safety requirements pertaining to the transportation industry.

• Skilled in establishing and maintaining effective working relationships with participants, caregivers, and co-workers.
• Proficient in identifying and recommending problem resolution.
• Ability to work independently with minimum supervision.
• Common sense problem-solving.
• Ability to utilize computers and other electronic devices.
