Job Description

The STREAM Clinical Informatics Specialist (CIS) is a position CIS will utilize clinical assessment skills, competencies, best practices along with new emerging technology to formulate holistic care plans that honor and uphold what matters most to the resident.
The use of innovative technology will allow for more accurate objective assessment data to be synchronized with current assessment data. The CIS and care team will then use this data, along with resident preference, to create individualized care planning strategies.
Understanding the significance that care planning along with resident needs has on quality of life will be a vital part of this role. The CIS will use their knowledge of unique late life care needs to support a balanced approach to care using maintenance and restorative care strategies with the goal of avoiding unnecessary decline while recognizing inevitable age-related decline.

Core Responsibilities
• Contribute to Work of Aging care planning with a balanced approach to biological, psychological and social late life needs of residents.
• Conduct assessments/observations to identify root cause of incontinence.
• Utilization of technological resources to assess needs (eg. bladder scanner, objective incontinence log, Actigraphy, etc.)
• Execute on-site Strategies Targeting Resident Elimination Assessment and Management (STREAM) initiatives with Empira direction and guidance.
• Increase awareness of the STREAM grant initiatives within the community.
• Partner and collaborate with staff development to provide education.

Job Requirements

Qualifications:

  1. Extensive experience in a health care setting; must have the knowledge and education necessary to understand internal and external customer needs in the health care environment.
  2. A bachelor’s degree in a health care related field Performance Improvement education/Analytics interpretation is preferred.
  3. Demonstrates an ability to utilize computer systems and applications.
  4. Demonstrates a strong belief in the importance or quality and acts as a role model for desired quality competencies. Commitment to following concepts of QAA and the QAPI process.
  5. Must have a valid Minnesota Registered Nurse licensure.
  6. Effective oral and written communication skills and teaching and coaching skills.
  7. Ability to work with unclear boundaries.
  8. Understanding of current Antibiotic Stewardship, Infection Prevention and Infection Control and OSHA guidelines.
  9. Knowledge of state and federal requirements for long term care.
