In this role you will be working both in our PACE clinic but mostly in our elderly participants homes.

This is a Monday through Friday position except for when on call.  Call is light.

PRIMARY RELATIONSHIPS: Home Care Coordinator, Nursing Director, Personal Care Service Manager, Personal Care scheduler, Center Director, Executive Director, Center staff, participants, family members and public.

OBJECTIVE: Under the direction of the Home Care Coordinator, provides in-home care to the participants of Senior CommUnity Care. As directed by the In–Home Services Supervisor performs initial or annual assessments of participants in their home care environment and coordinates plan of care with appropriate resources. Reviews plan of care and evaluate participant compliance. Provides skilled assessments, treatments and health education for participants in their homes, according to physician’s order. Maintains the participants’ medical record with thorough documentation. Communicates participants’ changes and progress to appropriate staff.


1. At the direction of the Home Care Coordinator, performs initial assessments of participants’ in their home environment to determine their individual needs and coordinates plan of care with care-giving resources. Coordinates with the Interdisciplinary Team to develop plan of care for each participant.

2. At the direction of the Home Care Coordinator, performs annual reassessments of all Senior CommUnity Care participants to determine that appropriate in-home services are provided and that participants are in the least restrictive environment.

3. At the direction of the Home Care Coordinator, performs quarterly supervisor visits to Home Health Aides.

4. Provides skilled supportive and restorative nursing care to participants as ordered, re-evaluating their plan of care and health status on an on-going basis.

5. Maintains participants’ medical record and fulfills agency charting and reporting requirements.

6. Re-evaluates the In-Home Service plan of care and monitors participants’ health status in home.

7. Provides skilled supportive and restorative nursing care to participants as ordered/requested.

8. Serves as a member of the Interdisciplinary Team, attends meetings as appropriate.

9. Monitors medication compliance, administers medications and provides training in self-administration of all non-scheduled meds.

10. Provides health counseling to participants and their families on an individual or group basis.

11. Participates in the coordination of personal care activities of all participants in the home.

12. Acts within the scope of his or her authority to practice.

13. Follows all Senior CommUnity Care policies and procedures and Occupational Safety Health Administration (OSHA) safety guidelines.
14. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

15. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

16. Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.

17. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

18. Performs other duties as required or requested.


Job Requirements

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before have direct participant contact.

2. Must have a valid driver’s license, proof of insurance and have means of transportation.

3. Education: Graduate of a school of professional nursing. Current Michigan Registered Nurses License, Cardio Pulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.

4. Experience: Experience working with the frail or elderly population required.

5. Skills/Knowledge:
• Broad knowledge base of health care problems of the frail elderly.
• Knowledge of medical equipment and instruments.
• Knowledge of common safety hazards and precautions to establish a safe working environment.
• Skilled in identifying problems and recommending solutions.
• Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
• Skilled in developing and maintaining department quality assurance.
• Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
• Ability to react calmly and effectively in emergency situations.
• Knowledge of home care services nursing principles and practices, with particular reference to the elderly.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individual with disabilities to meet these demands:

1. Physical Requirements – Requires a full range of motion including handling and lifting patients. Requires standing, walking, pushing, bending, kneeling and reaching at arms length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participants’ service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers.

3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.

Compliance with these safe work expectations will be required of all employees as a condition of employment. 1. Follow correct policy and procedures for department and facility Fire/Disaster/Missing Person Plan. Know the location of fire alarms and extinguishers.
2. Follow correct policy and procedure for hand washing.
3. Follow guidelines for proper method of lifting.
4. All doorways, hallways and areas are to be kept clear.
5. Know the location and purpose of Material Safety Data Sheets (MSDS).
6. Follow guidelines for department environment and safety measures.
7. Follow guidelines for safe handling, inspection, maintenance and storage of equipment. Report any malfunction of equipment.
8. Follow guidelines for safe handling and storage of chemicals and supplies.
9. Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
