In this role you will be working both in our PACE Senior Community Care Day Center caring for our elderly participants.

This position is full time at 64 hours every two weeks.

JOB SUMMARY: Provide direct primary medical care to all participants of the program. Works under the direct and indirect supervision of the Medical Director or designee but is independent in the application of advanced physician knowledge and skills.

1. Performs in person comprehensive history and physical on new Senior CommUnity Care of Michigan participants. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.

2. Conducts an in person reassessment semiannually and as needed.

3. Integrates the primary care treatment plan into the overall plan of care developed by the Interdisciplinary Team. Interacts with team members to meet emergent and acute need of participants. Participates in discharge planning for acute and long-term placement.

4. Evaluates and treats participants during acute illness.

5. As requested and directed by the Medical Director, manages care of participants in the nursing home: Provides regular visits as dictated by nursing home standards and participant need. Performs telephone contact with nursing home staff as required.

6. Maintains participants’ medical record and fulfills Senior CommUnity Care of Michigan charting and reporting requirements. Participates in monthly chart audit process; provides peer chart audit and consultation.

7. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.

8. Provides relevant participant and caregiver education. Educates and trains participant and caregivers in appropriate care and interventions. Actively participates in family conferences.

9. Provides staff in-service on a variety of relevant topics.

10. Completion and documentation of appropriate diagnostic coding.

11. Advises the Medical Director in ways and means to establish better accountability of Senior CommUnity Care of Michigan services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.

12. Assists with the development of, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.

13. Acts within the scope of his or her authority to practice.

14. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.

15. Follows all Senior CommUnity Care of Michigan policies and procedures and Occupational Safety and Health Agency (OSHA) safety guidelines.

16. Participates in continuing education classes and any required staff and training meetings. Maintains professional licensure and certifications.

17. Provides information about Senior CommUnity Care of Michigan Program to interested individuals and groups in adherence to PACE regulations.

18. Practices Universal Precautions and follows all appropriate infection control procedures.

19. Participates in and supports Quality Improvement Initiatives. Participates in team meetings, committees, and work groups as assigned.

21. Collaborates with SCCM NP’s in the assessment and development of a plan of care for SCCM participants.

22. Performs other duties as required or requested.

Job Requirements

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. Must have a valid driver’s license, proof of insurance and have means of transportation.

3. Education: Graduate of an accredited School of Medicine, Residency in Internal Medicine or Family Practice. Board certified in internal medicine, gerontology or family practice preferred. Additional training/Fellowship in Geriatrics preferred.

4. Experience: At least two (2) to three (3) years experience, and one year experience working with frail or elderly with significant chronic conditions.

5. Skills and Knowledge:
• Thorough knowledge of current concepts, theories and practices related to home and community-based care for the elderly and disabled adults.
• Working knowledge of health care delivery and financing systems, including Medical, Medicare, waiver programs, prospective and capitated systems, public health programs and Health Maintenance Organization/Managed Care.
• Working knowledge of the PACE regulations desired.
• Thorough knowledge of physical, mental and social needs of frail older adults.
• Effective oral and written communication skills.
• Ability to lead and work within the interdisciplinary setting.
• Strong organizational skills.
• Effective communication skills and strong community relations with other providers and organizations.
• Current license to practice medicine in Michigan.
• Valid driver’s license and reliable transportation.
• Proven clinical leadership. Ability to excel with others in a team environment. CPR certified.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
• Only acts within the scope of his or her authority to practice. Must meet a standardized set of competencies for the specific position description established by Senior Community Care of Michigan and approved by CMS before working independently.

1. Physical Requirements – Requires standing, walking, occasional pushing and pulling and lifting. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination. Must be able to document care provided in participants’ service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, caregivers, participants, and community at large.

3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.
