Job Description

Position Summary

Support the Assisted Living Administrator in the care and treatment of Memory Support residents entrusted to the community.  Serve as a liaison and partners with the Life Enrichment, Clinical, Dietary and other departments for the benefit of the Memory Support residents.  Responsible for the day-to-day programming and oversight of Memory Support operations.

Primary Relationships

Will interact directly with residents, their families/ responsible parties, and all levels of the organization.

Essential Duties and Responsibilities

The following duties are normal for this position.  This list is not to be construed as exclusive or all inclusive.  Other duties may be required and assigned.

 

  1. Conducts pre-admission and admission procedures for residents of Memory Care Community including psychosocial assessments and monthly documentation.
    1. Plans and initiates hospital or in-home visits with prospective residents on an as needed basis.
    2. Meets with new residents and assists with resident orientation and adjustment to their new environment; identifies and provides for the medically-related social, emotional, spiritual and psychological needs.
    3. Completes portions of the admission documents with resident, family member and/or responsible party.
    4. Advises residents and families of advance directives policies and procedures and partners with social service of Waterview Care Center and other internal departments or outside agencies as needed.
    5. Notifies Accounting Department of any changes in financial status of residents.

 

  1. Conducts pre-admission procedures, inquiries and tours on an as needed basis.

 

  1. Participates in routine, or requested, multi-disciplinary care conferences/family meetings held for residents.
    1. Provides support services and conducts or assists with therapeutic group work for residents and/or families when appropriate.
    2. Conducts or arranges for changes to the service plan and follows through on referred problems or psychological needs of residents.

 

  1. Partners with the Clinical Manager to evaluate the appropriateness of each placement, program content and to make recommendations on program services.

 

  1. Initiates reporting and documentation procedures for assigned residents.
    1. Maintains accurate and appropriate documentation in the medical records for Memory Support residents.
    2. Completes psychosocial assessment, i.e. social histories on new admissions; updates evaluations at regular intervals in accordance with Community policies.
    3. Prepares written summaries of family conferences, other related conferences, referrals made, contacts with residents and families and discharge planning in the Progress Notes in the medical record.
    4. Assists with the proper notification and documentation of resident transfers within the Community and/or to an alternate setting.
    5. Maintains accurate documentation on activities in the medical record.
    6. Track all behavior problems; works with nursing staff to alert physicians to behavioral challenges.
    7. Assists with resident discharge planning and proper documentation.

 

  1. Plans, directs, implements and documents activities and recreational programs that meet the specialized needs of the residents in the Memory Care Community.
    1. Plans and creates the activity calendar in collaboration with other departments.
    2. Plans, schedules and directs outside entertainment, outings, and events.
    3. Orders and processes invoices for the MS community per the AL Administrator.
    4. Assists the Life Enhancement Director in coordination of volunteers who help in the Memory Care Community.
    5. Continually reviews and revises the MS programming in collaboration with others to ensure resident engagement and differentiation with other memory support programs.

 

  1. Assists the AL Administrator with the development and promotion of positive public relations.
    1. Provides community understanding of Memory Care Community through a monthly support group for persons affected in some way by dementia.
    2. Provides Staff and family education opportunities.
    3. Represents the community at off-site events as needed or as directed by the AL Administrator.

 

  1. Collaborates with others related to the personnel functions of Memory Support.
    1. Partners with other Department Managers and Human Resource Manager on interviewing selection of associates to be assigned to Memory Support.
    2. Assists with providing all Associates in Memory Support with specialized training and orientation.
    3. Works with supervisors to discipline Associates on infractions of policies and procedures, recommends termination or transfers when needed.

 

  1. Monitors monthly program expenditure to budget and updates Spend Down spreadsheets.

 

  1. Other duties as assigned.

Job Requirements

QUALIFICATIONS:

Educational Requirements and Experience

  1. Bachelor’s degree in Social Work, Therapeutic Recreation, or related degree in social sciences, with current state licensure or 3 yrs. Experience in a dementia care setting preferred.
  2. Minimum of one year of experience, preferably in a long-term healthcare setting; has knowledge of medical and public health settings, principles of public welfare and knowledge of social and health resources of the community.
