PRIMARY RELATIONSHIPS:  Residents, family members, other staff members.

OBJECTIVE:  The primary purpose of the MDS Nurse is to plan and organize the MDS/RAI process in coordination with the RAI/PPS Coordinator.  This position assures compliance with all State and Federal MDS transmission requirements.  The MDS Nurse is responsible for collaborating with Clinical and Administrative staff regarding issues relative to the Resident Assessment Instrument (RAI) process.

  1. Current Florida Registered Nurse License
  2. Experience:
    1. Registered Nurse with previous long- term care experience with working knowledge of the RAI process preferred.
    2. MDS Coordinator previous experience required

A.      Resident Assessment Instrument Process

  • Works within coordination of RAI/PPS Coordinator for scheduling and proper coding of all new admission and PPS assessments.
  • Completes the minimum data set assessment (MDS) and Care Area Assessment (CAAS) within the required time frames.
  • Completes as assessment that is accurate with proper coding.
  • Consults with IDT members for pertinent clinical information as needed.
  • Utilize direct interview/observation skills, chart audits, resident/family interviews and communication with direct care staff when completing assessments.
  • Works with Interdisciplinary team to determine resident significant change in status and OMRA assessments.
  • Completes Nursing Sections of MDS per facility policy and ensures completion of assigned sections by other disciplines.
  • Follows and updates facility MDS schedule and tracking forms per policy and procedure.
  • Follow facility policy and procedure on Resident Care Planning.
  • Ensure that care plans accurately reflect the cares and clinical monitoring provided.
  • Audits completion and review of completed MDS assessments randomly on each floor monthly.
  • Distributes copies of audits for correction to appropriate departments.
  • Monitors regulatory compliance for completion of MDS/RAI process.
  • Notifies DON, MDS Coordinator and Executive Director and department supervisors of significant or persistent problem areas.
  • Coordinates the completion of Discharge Tracking Forms and Return Tracking Forms as required.
  • Tracks completion of MDS’s and transmits in compliance with HCFA timelines as delegated by RAI/PPS Coordinator or per facility policy.
  • Knowledge of the QRP process.
  • Reviews Validation reports and makes appropriate corrections.
  • Informs IDT members of validation errors as appropriate.
  • Follows the HCFA Correction Policy for corrections.


  1. Uses Quality Indicator data and other reports to improve MDS accuracy & compliance
  • Prints Quality Indicator Reports as delegated per policy.
  • Reviews & distributes as directed.
  • Assists IDT in determining MDS accuracy issues related to Quality Indicators.
  • With appropriate IDT members, develops and implements appropriate PIP’s through the QAA/QAPI process to correct MDS accuracy issues.


  1. Personnel Functions
  • Supports Nursing Administration in their absence. Participates in nursing on-call rotation per facility policy.
  • Attends ongoing educational offerings of topics pertaining to assigned duties to remain current in areas of responsibilities.
  • Attends mandatory in-services and meetings as assigned. Completes online educational modules as assigned.                
  • Provides educational training relating to Medicare, PPS, and MDS/RAI process to staff as requested.
  • Attends educational training relating to Medicare, PPS, and MDS/RAI process.

  5. Know the location and purpose of Safety Data Sheets (SDS).
  6. Follow guidelines for department environment and safety measures.
  7. Follow guidelines for safe handling, inspection, maintenance and storage of equipment. Report any malfunction of equipment.
  8. Follow guidelines for safe handling and storage of chemicals and supplies.
  9. Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
